If your insurance provider is not in our network,  direct payment is required.  We will provide you with statements for you to submitt to your insurance network.  We do not directly submit claims to carriers who are not in our network.

Fees

ADD/ADHD and Cognitive Functioning Assessment with QEEG:  $2,550

Our assessment of children and adults includes a thorough clinical history, and a battery of tests measuring Attention (auditory & visual), Psychological (non ADD specific), Neuro-psychological (brain functioning) and Cognitive/Learning & Memory (IQ, achievement, reading, language and expressive/receptive functioning).  Testing also includes a digital EEG analysis of brain wave patterns.  

A 90 minute diagnostic session with Dr. Johnson, all appropriate testing (includes face to face, take home, scoring & interpretation), a 90 minute follow up session with Dr. Johnson, complete written report with an individualized treatment plan is all included in the assessment.                                                                

 ADD/ADHD and Cognitive Functioning Consultation with QEEG:  $1,600

Consultation sessions are approved only if recently tested for ADD/ADHD (including visual/auditory CPT, IQ and Achievement) and seeking assistance with treatment planning. Includes interview, review of records, digital EEG, and a follow-up session.
